LIFE INSURANCE BENEFITS

Most life insurance companies require that the beneficiary of the policy furnish a completed form called a claimant's statement and a certified copy of the death certificate. If the policy is under two years old, they may require a form completed by the attending physician.

We at Wasson Funeral Home offer our assistance in helping our clientele process the paperwork without charge. We also will help obtain the certified copies of the death certificates.

Most insurance companies require the following:

  1. Surrender of the policy certificate or an affidavit attesting that the policy has been lost.
  2. Policy number
  3. Occupation and date last worked
  4. Date and place of birth
  5. Date and place of death
  6. Social Security number
  7. Claimant's name, date of birth, and Social Security Number

FINANCIAL AND CREDIT OBLIGATIONS

When a death occurs, certain financial housekeeping procedures should be done.
Banks and other financial institutions where the deceased person holds accounts or loans should be notified. All credit card companies should be notified.

Any installment credit or loans should be checked to determine if credit life insurance is included in the credit arrangement.

Following a death and funeral it is good business to have an attorney review a person’s affairs to see if the death has created changes that might affect a person’s will, trust, or property titles.
